Default 2010 STI, throwout bearing pops out of clutch

2010 STI, new engine, everything only has a couple thousand miles on it. 360/360 on the dyno

I installed this clutch (Competition Clutch Stage 2)

I used the provided throwout bearing. The other day I pushed the clutch pedal in and it went straight to the floor. Luckily I wasn't in heavy traffic so I got the car started and rev-match shifted to get it home.

Took off the intercooler, starter, and slave. Fork seemed stuck. I put a little pressure on it and it broke free, but flopped without any resistance. I was able to get a pry bar in the inspection hole and pop the throwout bearing back into the flywheel. Reassembled and the clutch felt fine without the car running. I cycled it several times. However after I started the car and pushed the clutch the bearing popped out again the first time I pressed the pedal.

I'm not too excited about pulling the engine again but whatever.

What can I do to prevent this from happening again? Does anyone use the ACT Monoloc? Did I just get a ****ty bearing? Is the center hole in my clutch made wrong?

Why would this happen after a few thousand miles? I'd think if something was made wrong it would fail right away. If it makes a difference, the clutch always made a little tiny bit of noise at idle, unless I put a bit of pressure on the pedal.

Any thoughts appreciated.

Only though is what I have had to do every time I had a car come in with this problem.
Pull it apart and inspect to determine the issue. Odds are something happened with the retainer and lock ring in the clutch cover (pressure plate).
Did you have trouble getting the engine and trans together? Not lined up properly when mating? Could have bumped the ring out of place...
