I enjoyed Allan McDonald's book far more than I did Gene Kranz's book.

The last chapter of McDonald's book talks about prior to the Columbia accident there were instances of debris impacts to the shuttle orbiter vehicle during launch.

During the initial investigation it was noted one of the boosters was recovered missing a foam fairing. The cold hard stare of blame once again swung toward the boosters. ATK was able to review the video of the booster splashdown and showed the missing fairing had not come off during flight and was jarred loose upon impact with the water.

Happy that the redesigned boosters were not to blame, NASA moved on. MacDonald was astonished that NASA closed the investigation at that point without sufficient answer to where the debris *had* come from.

The next mission was the one in which Columbia was lost.
